IMPORTANT HEALTH EXAM REQUIREMENTS
for STUDENTS ENROLLING in
MACOMB COMMUNITY UNIT SCHOOL DISTRICT 185

2009-20010 School Year

  • Physical examinations and immunizations are required for kindergarten, sixth, and ninth grade students and must be up-to-date according to the State of Illinois requirements PRIOR to the first day of school.  Students must provide proof that these requirements have been met prior to attending classes starting Friday, August 21, 2009, or they will not be able to attend until these have been completed.  New students to the district, regardless of grade level, have 30 days from the date of enrollment to meet these requirements..
  • Children entering kindergarten are now required to have an eye examination.  An exam up to one year prior to entering school is valid.  This must be completed by October 15, 2009. 
  • All children in kindergarten, second and sixth grades must present proof of dental examination prior to May 15, 2010. 

These exams are required by the State of Illinois Department of Public Health and enrolling students must meet these requirements.
